Our new dynamic supported living service Meridian Mews has been developed to create six contemporary supported living flats. Each flat has been designed to create a comfortable, safe and personal home for people with complex and challenging needs. All flats will be fully self-contained and provide each tenant with their own front door and individual garden space. Tenancy agreements will be placed and ‘easy-read’ or audio versions will be available for tenants who require them.
We will provide an enhanced support infrastructure that includes all multi-disciplinary team elements. Individuals can purchase specialised support from within our multi-disciplinary team in addition to the very skilled staff support available.
With the intensive support available individuals can positively address challenges within their lives and build their skills, confidence, self-esteem and social independence. It is likely to be important to create structure and pattern to each individual’s life, as people with a learning, mental health needs or Autism often require a very clearly defined structure that helps them maximise opportunities on a daily basis. Support provided will help individuals to carefully consider and create a daily structure where required.
All tenants will be empowered to be at the centre of plans and decisions within their lives. Working with support, their needs, wishes and aspirations will be captured and formed into a person-centred plan that directs the input they receive. Where individuals require specialist support in relation to their communication our Speech and Language team will develop ‘easy-read’ or other personalised means of presenting their plans.
Opportunities individuals might wish to develop and have included in their person-centred plan could include:
- Community engagement
- Money/budgeting skills
- Domestic/cooking skills
- Social groups
- Leisure pursuits
- Developing friendships
- Learning new skills
- Volunteering or work opportunities
