We operate Community Rehabilitation Hospital services which are located in the heart of local communities in North and South London. These services support individuals with a learning disabilities, mental health needs, autism and asperger’s conditions, and other associated needs that require higher levels of support.

For each individual we support in our Community Rehabilitation Hospital service, a Care Pathway will be developed. This is implemented through an initial assessment which details treatment and intervention plans and positive goals that Service Users will work towards as part of their journey with us. This Care Pathway develops as the goals are met and identifies what the future care and support package will look like to ensure the individual moves on to greater independence. This is the focus for the whole team.

All of our Staff are fully trained and provide compassionate and thoughtful support. The team are knowledgeable about mental health, learning disabilities and autism and asperger’s conditions. Sequence staff and Service users are in turn supported by our multi-disciplinary team of Consultant Psychiatrists, Mental Health and Learning Disability Nurses, Psychologists, Speech and Language Therapists, Occupational Therapists and other Therapists. This enables us to provide a fully holistic service to those we support.

Our services are fitted and finished to a very high standard with studio sized bedrooms, all with en-suites. We have communal kitchens, lounges and dining areas, which build a sense of community between Service users and ensure that a homely environment is maintained.

Our services are very close to the families of those we are supporting as well as the involved professionals and placing authority. This ensures that the individuals we support benefit from high levels of input from their circles of support and regular visits to the service by family members or visits back to the family home. The community location ensures we can support people to be part of their community accessing Occupational, Educational and Leisure facilities.

The outcomes we achieve are testament to the hard work of our staff and Service users, with individuals moving on to lesser restrictive environments in an average period of 12 to 18 months.

Over 92% of individuals moving out of our services have moved back home, on to residential services or, in the majority of instances, through to supported living.
